I-90 Checklist: What to Have Ready Before Starting
Before completing the I-90 intake, it helps to have the following information and documents ready. Some items may not apply to your situation. This checklist is only to help you prepare your information. Basic Card Information
Your current Green Card, expired Green Card, or the incorrect/damaged card you are replacing. A clear front-and-back copy is ideal.
Your exact legal name and personal information exactly as shown on your current card.
Your A-Number.
Your class of admission, date of admission, and the place where you applied for your immigrant visa or adjustment of status, if available.
Personal and Address Information
Your full mailing address.
Your physical address, if different.
Your mother’s and father’s names.
Your date and country of birth.
Documents, If They Apply
Any legal name-change document, such as a marriage certificate, divorce decree, or court order, if applicable.
Any document related to your filing reason. For example: your current or expired card for renewal, the incorrect card for a USCIS or biographic correction, or your name-change document for a new legal name.
Full English translations with translator certification for any non-English document.
